Here is a handy tip for a walk around Vyšehrad: the Visitor Center on V Pevnosti street has well-maintained toilets with a baby-changing table. It is a convenient place to change your baby before continuing your Prague adventures. During our visit, toilet entry cost CZK 20. The map in this post marks the Visitor Center.

If you only need a toilet, we found a free alternative at a cultural centre with a restaurant near the ramparts and a beautiful view across Prague. There is no baby-changing table there, though, so the Visitor Center is the more practical option for changing your baby.
